Breast implant removal

The procedure is always preceded by an individually tailored treatment plan. Taking into account the patient’s anatomy, previous breast surgery, the type and condition of the implants, and the patient’s expectations, the plastic surgeon determines the most appropriate surgical approach. In certain cases, implant removal may be combined with removal of the scar tissue capsule surrounding the implant or with a breast lift.

When is breast implant removal recommended?

There are various reasons why breast implants may be removed. Some patients decide after many years that they no longer wish to have implants, while for others, the procedure may become necessary for aesthetic or medical reasons.

Common reasons for breast implant removal may include:

• the desire to permanently remove the implants,
• implant damage or rupture,
• capsular contracture, which may cause breast firmness, tightness or changes in breast shape,
• pain or other implant-related symptoms,
• other changes or abnormalities around the implant,
• changes in the position of the implant or the shape of the breast,
• changing personal circumstances or aesthetic preferences that mean the patient no longer wishes to have breast implants.

Implant removal does not necessarily mean that the breasts will return to their original size and shape. Depending on the size of the implants, how long they have been in place, the effects of pregnancy and changes in body weight, as well as individual tissue characteristics, the skin and breast tissue may have changed to varying degrees. During the consultation, we therefore also discuss whether an additional breast procedure may be appropriate.

What happens during the initial consultation?

During the consultation, our plastic surgeon assesses the current condition of the breasts, the position of the implants, as well as any scars from previous surgery and the capsule that has formed around the implants. We discuss the result the patient would like to achieve following implant removal and the available treatment options. Our aim is always to ensure that patients have realistic expectations of the changes they can expect before undergoing surgery.

The consultation also determines whether implant removal alone is sufficient, or whether partial or complete removal of the surrounding capsule and/or a breast lift may be appropriate to achieve the desired result.

Preparing for breast implant removal

As with any surgical procedure, a comprehensive medical assessment is required before breast implant removal. This may include laboratory tests, an ECG where indicated, and imaging examinations such as breast ultrasound, mammography or MRI.

What happens during the surgery?

Breast implant removal is performed under general anaesthesia. Whenever possible, the plastic surgeon uses the existing surgical scar to make the necessary incision, then accesses and removes the implant.

The exact surgical technique depends on the condition of the implant and the capsule that has formed around it. In some cases, the implant can be removed on its own, while in others, partial or complete removal of the surrounding scar tissue capsule may also be necessary. The appropriate approach is determined by the plastic surgeon based on the individual circumstances of each patient.

How much pain can be expected after breast implant removal?

Following surgery, patients may experience tenderness, tightness and mild to moderate pain in the treated area. The level of discomfort may depend, among other factors, on whether implant removal was performed alone or combined with capsule removal and/or a breast lift.

Postoperative pain can generally be well controlled with appropriate medication. Following surgery, our patients receive detailed instructions regarding pain management, wound care and follow-up appointments.

Recovery and healing

Following breast implant removal, our patients stay overnight at our private hospital, allowing them to rest under medical supervision during the initial postoperative period. The following day, our plastic surgeon examines the surgical site and provides detailed instructions for the next stage of recovery.

Strenuous physical activity should be avoided during the early stages of recovery. Depending on the type of surgery performed and the individual healing process, patients can generally return to work and their usual daily activities within 2-4 weeks.

A special postoperative support bra should be worn after surgery, typically for 4-6 weeks.

The scars heal gradually. They may initially be more visible before becoming increasingly less noticeable over time. Scar healing varies from person to person, and it may take several months for the final appearance of the scars to develop.

What will the breasts look like after implant removal?

Breast shape and size will change following implant removal. The final result is influenced by several factors, including the size of the implants, how long they have been in place, skin elasticity, the amount of natural breast tissue and the degree of any existing breast sagging.

After implant removal, the breasts may appear less full or more sagging than before. If the patient wishes to address this, breast implant removal can be combined with a breast lift.

An important part of the consultation is therefore discussing whether implant removal alone or implant removal combined with a breast lift is more likely to achieve the desired breast shape.
