When is hydrocele surgery necessary?

A build-up of serous fluid in the scrotum, called a hydrocele – which in some cases can be accompanied by or develop into a hernia - is often a congenital disorder, but it can also develop due to inflammation, injury or cancer. In many cases the cause of the disease is unknown. A hydrocele may, depending on the size and extent of the swelling, cause pain or a feeling of heaviness, and in the long term it can lead to testicular damage. Treatment of a hydrocele by aspiration of the fluid using a needle or syringe is only effective in the short term because the fluid rapidly builds up once again. For long-term results, surgery is recommended. 

How to prepare for hydrocele surgery?

The procedure requires no special preparation on the part of the patient.

What preliminary tests are required? 

In addition to a urological examination, a laboratory test and an anesthetic consultation are required before the procedure.

How is hydrocele surgery performed? 

The surgery is performed under local anesthesia, general anesthesia or spinal anesthesia. The testicle is accessed by an incision in the scrotum or in the groin, in order to open and drain the fluid-filled sac. By suturing the inverted or reduced-sized sac, the recurrence of the hydrocele can usually be prevented.

What should the patient do after hydrocele surgery?

An overnight stay after the operation is required. After the surgery, it is recommended to keep the testicle well supported and apply local cooling for 1-2 weeks. It is important to drink adequate amounts of fluids. Physical exertion should be avoided for 4 weeks. The hydrocele operation has no effect on general physical condition or sexual function, so normal life can be resumed.

What are the risks of hydrocele surgery?

After surgery, there may be tissue swelling and hematoma, but these are usually temporary. Inflammatory complications, hospital-acquired infections and deep vein thrombosis may occur, however the chances of these are minimal if proper protocols are followed. The chance of recurrence of a post-operative testicular hernia is less than 5%.
