CT-guided nerve root block

A CT-guided nerve root block is a targeted, minimally invasive pain-relieving procedure. Its purpose is to deliver anti-inflammatory medication and a local anaesthetic into the area surrounding a nerve root that exits the spine and is inflamed or under pressure. The procedure is performed using CT (computed tomography) imaging, which allows the physician to guide the needle to the appropriate location with a high degree of precision. The treatment does not eliminate the underlying condition in every case (such as a herniated disc); however, it can significantly reduce pain and inflammation, thereby improving quality of life and supporting rehabilitation.

In which cases may the procedure be required?

Your doctor may recommend a CT-guided nerve root block in the following cases, among others:

  • nerve root pain caused by a herniated disc;
  • sciatica (pain radiating along the sciatic nerve);
  • pain radiating into the leg or arm;
  • neurological symptoms associated with spinal canal stenosis;
  • recurring or persistent pain following spinal surgery;
  • suspected recurrence of a disc herniation;
  • for diagnostic purposes, when it is necessary to determine which nerve root is responsible for the symptoms.

How is a CT-guided nerve root block performed?

Preparation

Before the treatment, your doctor will review your medical history, current medications, and the results of any previous imaging examinations.

It is particularly important to inform your doctor about:

  • any blood-thinning medications you are taking;
  • any known allergies to medications or contrast agents;
  • diabetes;
  • any existing infection or fever;
  • possible pregnancy.

The procedure

The patient lies face down on the CT table. After disinfecting the treatment area, the physician uses CT images to guide a thin needle with millimetre precision to the affected nerve root. If necessary, a small amount of contrast agent is used to confirm the correct position, after which a medication containing a local anaesthetic and an anti-inflammatory agent is administered.

The entire procedure usually takes 5-10 minutes.

Afterwards, the patient is monitored for a short period, and if their condition is appropriate, they may leave the clinic and return home.

What may you feel during the procedure?

Most patients experience only mild discomfort.

The following may occur:

  • a brief stinging sensation when the local anaesthetic is administered;
  • a feeling of pressure;
  • radiation of the usual pain into the limb for a few seconds;
  • temporary numbness or a sensation of warmth.

These sensations usually last only a short time.

What can be expected after the treatment?

Pain relief may occur:

  • within a few hours;
  • after a few days;
  • or gradually over a period of 1-2 weeks.

During the first 24-48 hours, pain may temporarily increase, which does not necessarily indicate a complication.

The effect of the treatment varies from patient to patient. In some cases, a single treatment is sufficient, while in rare cases multiple sessions may be required. In addition, other conservative (non-surgical) treatment methods are often recommended, such as anti-inflammatory and muscle relaxant medication or infusion therapy, personalised physiotherapy, massage, and physical therapy.

What are the benefits of a CT-guided nerve root block?

The aim of the treatment is to:

  • reduce pain;
  • decrease inflammation around the affected nerve root;
  • improve mobility;
  • reduce the need for pain-relieving medication;
  • enhance the effectiveness of physiotherapy;
  • determine whether the specific nerve root is indeed responsible for the symptoms.

What are the possible side effects and risks of the procedure?

As with any medical procedure, a CT-guided nerve root block may involve potential side effects and complications.

Common, usually mild side effects

  • temporary increase in pain;
  • tenderness at the injection site;
  • minor bruising;
  • redness of the skin;
  • temporary numbness or muscle weakness;
  • temporary increase in blood sugar levels in patients with diabetes.

Rare but important risks

  • infection;
  • bleeding or the formation of a haematoma;
  • nerve injury;
  • allergic reaction to the medication or contrast agent;
  • very rarely, a severe allergic (anaphylactic) reaction.

With appropriate preparation and sterile technique, these complications are very rare.

What should you pay attention to after the procedure?

On the day of the treatment, it is recommended to:

  • arrive and leave with an accompanying person;
  • avoid driving for 8 hours following the procedure;
  • rest;
  • avoid strenuous physical activity on the same day.

Seek medical attention immediately if you experience:

  • high fever;
  • increasing, unbearable pain;
  • significant weakness or paralysis;
  • difficulty urinating or problems with bowel movements;
  • shortness of breath or severe allergic symptoms.

Who is the procedure not recommended for?

A CT-guided nerve root block is generally not recommended in the following cases:

  • untreated blood clotting disorders;
  • inadequately controlled anticoagulant therapy;
  • the presence of an active infection;
  • skin infection at the injection site;
  • known severe allergy to medication or contrast agents (without appropriate preparation);
  • during pregnancy, unless after individual medical assessment and consideration.
