Ovarian surgery can be divided into two main groups: operations to treat benign and malignant lesions. Malignant ovarian lesions are treated in Hungary in a centralized system in specialized centers. In all cases where the possibility of malignancy is raised, surgery can only be performed in these centers.

What are the treatment options for benign ovarian lesions?

At Dr. Rose Private Hospital, we primarily provide surgical treatment for benign ovarian lesions. These are predominantly divided into two groups:

  • Ovarian cysts and cystic lesions of the ovary.
  • Endometriosis, or dermoid cyst-type lesions.

During surgical procedures, our basic aim is to maximize organ preservation, i.e., to remove as little intact ovarian tissue as possible. However, in some cases, removal of the entire ovary is unavoidable if the lesion affects the whole organ. In such cases, the other, intact ovary is fully able to take over hormone production and reproductive functions.

What tests are needed before ovarian surgery?

Accurate diagnosis of ovarian lesions is essential to develop an appropriate treatment plan. Key tests include:

  • Tumor marker tests 
  • Color Doppler ultrasound
  • Pelvic contrast-enhanced MRI scan

These examinations help to determine whether the lesion is benign or malignant. If the possibility of a malignant lesion can be ruled out and surgery for a benign lesion is performed, a general preoperative check-up is required, which includes laboratory tests, and a chest x-ray and an ECG if you are over 60 and a smoker. These tests are very important for anesthesia.

Usually, if the surgical procedure is not urgent, it is not performed during menstruation, but rather between the two menstrual cycles.

How does ovarian surgery take place?

Most ovarian surgeries are performed using a minimally invasive procedure called laparoscopy. This has the advantage of faster postoperative recovery, less pain and shorter hospital stays, hence it is the preferred method. If a large cyst size or other anatomical constraints do not allow laparoscopy, open abdominal surgery is used. In some cases, laparoscopy may have to be converted to open surgery if removal of the lesion is not possible via minimally invasive methods.

The course of the ovarian surgery is essentially determined by whether it is possible to keep the ovaries intact or whether it is necessary to remove the whole ovary, and sometimes the fallopian tubes. Ideally, the lesion is removed from the surface of the ovary or its surroundings, which determines the course of the operation. In many cases, the final surgical plan is only formulated during the operation, when we can directly see the nature of the lesion and adapt the intervention to the specific conditions.

What should you do after ovarian surgery?

After the laparoscopic procedure, our patients spend one night in our institution under observation. The result of the histological examination is expected within two weeks, and the patient is expected back for a follow-up examination. The total recovery time for laparoscopy is approximately 3 weeks. During this time, it is particularly important to spare yourself from physical exertion and avoid both sexual activity and submerged bathing. If the specialist finds all is well at the check-up, the patient can resume their normal lifestyle.

In the case of open abdominal surgery, a longer recovery time should be expected. The total recovery time is 5-6 weeks, which should be spent resting, with a check-up in the sixth week, following which the restrictions regarding physical exertion, sexual activity and submerged bathing, are lifted.

What complications can ovarian surgery have?

As with all surgical procedures, there are risks associated with ovarian surgery.

Among the short-term complications, bleeding complications must always be considered, which may result from postoperative bleeding from the wound or surgical site and require hospitalization. Should any injury occur during the procedure, whether to the abdominal wall or organs, it will be resolved during the surgery.

Long-term complications may include inflammation of the wound area, which is symptomatic and causes a noticeable lesion. In such cases, appropriate treatment should be carried out at a later date.
