When is bladder tumor removal surgery necessary? 

TURBT (transurethral bladder tumor resection) surgery, i.e., the removal of a bladder tumor through the urethra, may be necessary for bladder tumors or other bladder lesions. 

One of the most common symptoms of bladder cancer is painless bloody urine (hematuria), which occurs in 85-90% of cases and is usually intermittent.

In rarer cases, symptoms of bladder irritation may also occur, which can include:

  • an urgent need to urinate,
  • frequent urination (pollakiuria),
  •  painful urination (dysuria).

It is important to note that the incidence of asymptomatic, superficial bladder tumors is relatively low. Most bladder tumors can be detected by abdominal ultrasound, but bladder ultrasonography may be necessary to identify smaller lesions and to confirm or exclude lesions detected by ultrasound.

How should the patient prepare for bladder tumor removal surgery?

The procedure requires no special preparation on the part of the patient.

What preliminary tests are required? 

In addition to the urological examination, a laboratory examination and an anesthesiology consultation are required.

How is TURBT surgery performed? 

The surgery is performed under spinal anesthesia, but general anesthesia can also be used. During the operation, a flexible bladder scope (resectoscope) is inserted into the bladder through the urethra, so no incision is needed. The bladder tumor is removed layer by layer, and the excised tissue is then examined histologically. After the procedure, a permanent bladder catheter is inserted after the bleeding has stopped. If necessary, at the end of the operation, chemotherapy drugs are injected into the bladder through the bladder catheter to act locally in the bladder and reduce the chance of recurrence.

What should the patient do after bladder tumor removal surgery?

After the procedure, a one-night stay is usually required. After 1-2 days following the operation, or 2-4 days for larger tumors, the bladder catheter that was inserted during the surgery is removed. It is important to drink adequate amounts of fluids after the operation. Physical exertion should be avoided for 4-6 weeks. 

What are the risks of TURBT surgery?

After the operation, you may experience post-operative bleeding, irritative symptoms and urinary retention problems, but these are usually temporary. Inflammatory complications, hospital-acquired infections and deep vein thrombosis may occur, but the chances of these are minimal if proper protocols are followed.
