When is surgical circumcision necessary?

Surgical circumcision may be performed in cases of constricted foreskin, foreskin pinching, chronic inflammation of the glans, precancerous conditions on the glans or foreskin, and at the patient's request for religious or non-religious reasons. A constricted foreskin may be congenital, caused by viral infection and metabolic disorders, or may be of unknown origin. Circumcision involves the surgical removal of the foreskin that covers and protects the end of the penis (glans). After the surgery the glans can be more easily cleaned, and the procedure eliminates the complaints caused by the constriction, including those involving discharge, skin complaints of the foreskin, as well as sexual and urinary complaints. The surgery reduces the risk of penile cancer. Surgical circumcision is irreversible, thus providing a long-term solution.

Surgical circumcision can be performed from the age of 5 years if the patient’s bodyweight is at least 17kg.

How should the patient prepare for surgical circumcision?

The procedure does not require any special preparation on the part of the patient.

What preliminary examinations are required?

No examination other than a urological examination is required prior to the procedure.

How is circumcision surgery performed?

The surgery is performed under local anesthesia or general anesthesia. The sore, narrow-apertured foreskin is incised longitudinally along the upper circumference, to the level of the glans, and then the scarred inner and outer skin is excised, leaving a 3-4mm portion behind. The wound edges are then stitched with absorbable sutures.

What should the patient do after surgical circumcision?

After the procedure, a 3-4-hour stay is required. The dressing can be removed the day after surgery. The wound should then be disinfected daily, bathed with lukewarm chamomile extract, and then carefully wiped dry. After the dressing is removed, no further dressing is required. The wound will heal in about 2 weeks. After the surgery, sexual intercourse, submerged bathing, and swimming pool use are not recommended for about 4 weeks.

What are the risks of surgical circumcision?

Painful swelling, infection and bleeding may occur after surgery, but these are typically temporary. In some cases, wound healing may be impaired, which is more common with comorbidities (systemic sclerosis, diabetes). In addition, the wound may open, which can retard healing. Scarring due to the surgical incision can cause aesthetic or functional problems. Inflammatory complications, hospital-acquired infections, and deep vein thrombosis may occur, but the chances of these are minimal if the appropriate protocols are followed.
