Hysterectomy surgery can have several indications. Basically, a distinction is made between benign and malignant lesions, and also between surgery to address increased bleeding problems or other problems.

What tests are required before hysterectomy surgery?

Before hysterectomy surgery, a gynecological examination and an ultrasound scan are necessary. Other imaging procedures such as CT or MRI scans may be required prior to surgery, as well as laboratory tests, chest x-rays, ECGs and anesthetic consultation as part of the general surgical examination. If the patient has an underlying medical condition, a consultation with an internist will also be carried out.

What types of hysterectomies are there?

There are several types of hysterectomy. There is a total hysterectomy, which means that both the uterus and the cervix are removed, or a partial hysterectomy, where only the uterus is removed. Today, hysterectomies are usually performed with removal of the fallopian tubes on both sides to prevent malignant lesions.

What determines whether an abdominal or vaginal hysterectomy is performed, and in which cases is a laparoscopic procedure performed?

For each patient, we decide on an individual basis whether to perform the operation abdominally, laparoscopically or vaginally. For a vaginal hysterectomy, the ligaments must be sufficiently mobile to enable this procedure. For laparoscopic or open abdominal surgery, it is usually the indication for the operation that decides whether laparoscopic or open abdominal surgery is recommended.

How should I prepare for hysterectomy surgery?

Hysterectomy surgery is performed outside the menstrual period. The patient should time the post-operative recovery period so that it is entirely restful.

How is hysterectomy surgery performed?

Most hysterectomies are performed under general anesthetic, but spinal anesthesia may be used for vaginal hysterectomies. An abdominal hysterectomy is performed via an abdominal incision. For a vaginal hysterectomy, the uterus is removed through the vagina. In all cases, the operation is followed by preventive antibiotic treatment.

What should patients do after hysterectomy surgery?

After a vaginal or an abdominal hysterectomy, a follow-up examination is carried out six weeks after the operation, but the six-week period should be spent resting at home and recovering in complete peace and quiet, as in a hospital. Physical exertion or any other activity that could cause either suture failure or late complications should be avoided. Sexual activity and submerged bathing should also be avoided after surgery. Anti-thrombotic blood thinning treatment should be continued for the time prescribed by the doctor. 

In the case of an abdominal hysterectomy, the wound does not require treatment due to the absorbable sutures and the suturing technique, but the dressing applied in the operating theater should be removed after a few days.

What are the risks of hysterectomy surgery?

Both vaginal and abdominal hysterectomies are major gynecological operations and are therefore very demanding on the body. There are three main types of complications: bleeding complications, inflammatory complications and complications from wound healing. The chances of these complications are minimized with preoperative and postoperative antibiotic treatment and disinfection according to protocol.
