Loop conization of the cervix

Loop conization, also known as a cone biopsy, is a special cervical surgery that can be used for both therapeutic and diagnostic purposes. During the procedure, a persistent cervical wound that does not heal with conservative treatment and causes inflammatory vaginal complaints is repaired for therapeutic purposes. High-risk HPV infection or abnormal cytological results may also indicate surgery, in which case the procedure also serves diagnostic purposes. In this case, the affected area of the cervix is removed and sent for histological examination to accurately determine the condition of the cells. The histological result determines whether further treatment is necessary or whether surgery alone is sufficient to solve the problem.

What examinations are required before loop conization surgery?

Before the procedure, in addition to a gynecological examination, a laboratory examination and an anesthesiology consultation are required, and in certain cases an ECG, chest x-ray and internal medicine consultation may also be necessary.

How to prepare for loop conization surgery?

Loop conization should be scheduled after menstruation, as one of the possible complications of the procedure is postoperative bleeding. If the surgery is performed in the days leading up to menstruation, bleeding that may occur as a complication of the surgery would be more difficult to distinguish from menstrual bleeding.

Also, you should be prepared in advance for about two weeks of rest and physical restraint after surgery.

How does loop conization take place?

Loop conization is performed under intravenous anesthesia. At the beginning of the operation, the vagina is disinfected before the loop electrosurgical instrument is used to remove the affected part of the uterine cavity. If minor bleeding occurs during the procedure, we immediately stanch it. At the end of the operation, the vagina is disinfected again to ensure that the wound heals properly and to prevent infection.

What should the patient do after a loop conization?

After the procedure, our patients usually leave following 5-6 hours of observation. The total recovery time after loop conization is approximately 3-4 weeks. During the procedure, a scar is created on the uterine wall, and rest and relaxation are of paramount importance during the first two weeks to ensure a full recovery. During this period, strenuous exercise, lifting heavy objects, sexual intercourse and submerged bathing should all be avoided as these can hinder proper wound healing and increase the risk of postoperative bleeding. The removed tissue will always be sent for histological examination, the results of which will be available within two weeks. At the postoperative check-up, the doctor will decide whether further treatment is necessary. If the wound is healing properly and there is no further bleeding, the patient can gradually resume their regular lifestyle.

What are the possible complications of loop conization?

Like all surgical procedures, loop conization can be associated with bleeding, injury and inflammatory complications. With antibiotic treatment before and after surgery and disinfection according to protocol, the chances of these occurring are minimal.
